Life Insurance Replacement Declaration Form
The harmonized Life Insurance Replacement Declaration (LIRD) form, developed in 2008, has been adopted in several jurisdictions and others are in the process of adopting the new form. Please ensure that the form is acceptable in your jurisdiction before using it.
Agent & Broker Applications
Insurance agents or brokers currently licensed in a Canadian jurisdiction and applying for a non-resident licence in another Canadian jurisdiction (except the Yukon) can utilize the application forms that have been jointly developed by the Canadian Council of Insurance Regulators (CCIR) and the Canadian Insurance Services Regulatory Organizations (CISRO).

Instructions that provide direction on how to complete the application, and
Appendix A providing jurisdiction specific requirements along with which documents must be submitted with the application.
There is also an Appendix B which provides the mailing addresses and websites for the licensing authorities in all Canadian jurisdictions.
Some jurisdictions have also agreed to accept the application forms from residents applying to become licensed in their home jurisdiction. Currently, Ontario Northwest Territories, Nunavut, and Prince Edward Island have also agreed to accept the applications from residents.
In 1998, CCIR developed a standardized Application for Insurer's License form. The form is used by insurers seeking incorporation and/or a license to undertake the business of insurance in a Canadian jurisdiction. It is updated periodically.
