Application
Forms for Accident and Sickness, Life and General Insurance Agent and
Broker Licensing

Insurance agents
or brokers currently licensed in a Canadian jurisdiction and
applying for a non-resident licence in another Canadian
jurisdiction (except the Yukon) can utilize the application forms
that have been jointly developed by the Canadian Council of
Insurance Regulators (CCIR) and the Canadian Insurance Services
Regulatory Organizations (CISRO).
There are two
sets of application packages:
Each package
consists of:
- Application
Form;
- Instructions
that provide direction on how to complete the application; and
- Appendix
A providing jurisdiction specific requirements along with
which documents must be submitted with the application.
There is also an
Appendix B which provides the mailing addresses and websites for
the licensing authorities in all Canadian jurisdictions.
Some
jurisdictions have also agreed to accept the application forms
from residents applying to become licensed in their home
jurisdiction. Currently,
Ontario Northwest Territories, Nunavut, and Prince Edward Island
have also agreed to accept the applications from residents.
